腹腔镜胆总管探查术临床应用分析

目的：探讨腹腔镜胆总管探查术治疗胆总管结石病疗效和应用价值。方法：对我院2001年1月至2007年8月实施的387例腹腔镜胆道探查术进行回顾性分析。结果：385例患者行腹腔镜胆总管探查术成功，2例中转开腹，成功率99．7％；手术时间60～230min，平均85min。术后发生胆漏13例，十二指肠损伤1例，腹腔残石致引流管口脓肿形成1例，胆管残石35例，T管引流出钛夹3例。387例全部治愈，随访6—24个月无并发症，健康良好。结论：腹腔镜胆总管探查术住院时间短、痛苦少、恢复快，只要具备娴熟的腹腔镜操作技术和腹腔镜下胆道镜技术，它是治疗胆总管结石可行、安全、有效的方法。

Clinical Application Analysis of Laparoscopic Common Bile Duct Exploration

Objective to study the laparoscopic common bile duct exploration for treatment of choledocholithiasis and its efficacy and application value. Methods from January 2001 to August 2007 the implementation of the 387 cases of laparoscopic bile duct exploration were reviewed. Results 385 cases of patients with laparoscopic common bile duct exploration were successful, and 2 cases of conversion to open laparotomy, the success rate of 99.7 percent; operation time was 60 - 230min, an average of 85min. Postoperative bile leakage occurred in 13 cases, 1 case of duodenal injury, the abdominal cavity caused by residual stone drainage pipe mouth abscess formation 1 cases, 35 cases of bile duct stone residue, T-tube drainage of titanium folder 3. 387 cases were cured, the follow-up of 6-24 months, without complications, in good health. Conclusion laparoscopic common bile duct exploration short hospital stay, less pain and rapid recovery, as long as skilled with the technique of laparoscopy and laparoscopic biliary microscopy, which is the treatment of common bile duct stones feasible, safe and effective.
