支架辅助机械解脱弹簧圈栓塞治疗颅内宽颈动脉瘤的临床应用

摘    要:目的:探讨支架辅助机械解脱弹簧圈栓塞治疗颅内宽颈动脉瘤中的安全性、疗效、技术优势及临床应用价值。方法:采用Seldinger穿刺技术,应用颅内支架辅助瘤颈成形术结合AxiumTM弹簧圈栓塞治疗颅内宽颈动脉瘤26例。结果:所有动脉瘤均成功植入支架,其中Boston公司的Neuroform支架19枚、Cordis公司的Enterprise支架7枚。术后即可完全填塞20例,大部分填塞6例,载瘤动脉通畅。术后随访6~12个月,未发现出血或脑血栓形成。结论:联合使用支架辅助机械弹簧圈栓塞是治疗颅内宽颈动脉瘤安全有效的方法,支架后释放技术显著提高动脉瘤颈覆盖率而达到血管重建目的,使宽颈动脉瘤的介入治疗成为可能,使颅内动脉瘤介入治疗技术成功率、安全性和疗效得到进一步提高。

CLINICAL APPLICATION IF STENT-ASSISTED MECHANICALLY-DETECHABLE COIL EMBOLIXATION IN TREATING INTRACRANIAL WILD-NECKED ANEURYSMS

Abstract:Objective:To discuss the safety,efficacy and advantages,the clinical application of stent-assisted mechanically-detechable coil embolization in treating intracranial wild-necked aneurysms.Method:Twenty-six patients with wild-necked aneurysms were treated with embolization therapy by stent-assisted technique combined with the use of mechanically-detechable AxiumTMcoil through Seldinger technique.Results:Stent placement successed Technically in all of the wild-necked aneurysms,including 19 Boston Neuroform stent,7Cordis Enterprise stent.Total occlusion was achieved immediately in 20 cases and more than 90% in 6 cases,the patients recovered well with patency of the parent arteries.All the patients recovered well.Neither rebleeding nor thrombosis-related sympotoms occurred during a follow-up period of 6-12months.Conclusion:For the treatment of intracranial wild-necked aneurysms,stent-assisted mechanically-detechable coil embolization technique is safe and effective.Stenting after coiling techniques can facilitate the improvement of neck forvascular reconstruction,and technical successful rate,safety and efficacy.
