摘    要:
目的探讨畸变产物耳声发射应用于新生儿缺氧缺血性脑病听力随访的可行性,从临床角度早期干预有听力损害高危因素的新生儿。方法对35例正常新生儿和45例缺氧缺血性脑病(HIE)新生儿在出生后进行畸变产物耳声发射(DPOAE)检查,对HIE新生儿1,6个月后DPOAE及听性脑干反应(ABR)听力随访。结果HIE组初查,1,6个月复查DPOAE通过率分别为67%,73%,80%;HIE组1个月复查2000Hz,HIE6个月复查2000,4000Hz的通过率高于HIE组初查;有2例HIE新生儿ABR确诊为听力损害。结论DPOAE可作为听力损害的高危因素HIE新生儿听力随访的一种有效方法;HIE新生儿2000,4000Hz听力有改善的可能。

Abstract:
Aim To investigate the feasibility and c linical significance of hearing test using distortion produ ct otoacoustic emissions(DPOAE)when it is applied to the follow-up of neonates with hypoxic ischemic encephalop athy(HIE).Methods 35healthy newborns and 45cases of HI E neonates were tested by DPOAE before their discharge of the hospital.All HIE neonates were tested with DPOAE 1and 6months l ater.Those who failed in 2tests with DPOAE were tested with brain-stem response(ABR).The results were compared with those of 35healthy neo nates using prospective cohort.Result The presence of DPOAE in HIE neonates was 67%in the first test.In the follow-up one and six months later,t he presence was 73%and 79%,re-spectively.The presence of DPOAE at2000Hz in the first month and at both 2000Hz and 4000Hz in the 6month were higher than that in the first test.Two HIE neonates were diagnose d as hearing loss by ABR.Conclusion The DPOAE test has been found to be an e fficient way in the follow-up of HIE neonates who are considered as hi gh risk candidates for hearing loss.It is possible that the hearing of HIE neonates could be improved at 2000Hz and 4000Hz.
