颧骨弧形牵张成骨修复部分上颌骨缺损的体外模型模拟研究

摘    要:目的:考察应用自行研制的骨牵张器实现颧骨弧形牵张成骨修复部分上颌骨缺损的可行性.方法:在1例树脂人体头颅模型标本上进行颧骨弧形牵张成骨修复部分上颌骨缺损的体外模型模拟研究.首先,切除右侧大部分上颌骨以制造上颌骨缺损,在缺损区后外侧制作宽约17 mm的骨转移盘并分别安置弧形及直线骨牵张器.随后,以加力杆旋转中心螺杆,驱动骨转移盘沿轨道运动.结果:颧骨内置弧形骨牵张器与人体颧骨、上颌骨区域骨性结构匹配性良好,颧骨转移盘在该牵张器作用下弧形运动约20 mm到达预定部位,骨转移盘与对侧上颌骨颧突对称、协调.结论:上颌骨大型缺损时可利用颧骨内置式弧形牵张成骨技术在修复面容的同时,修复部分上颌骨,重建骨性支持,为后期功能性修复奠定良好基础.

Abstract:Objective:The aim of the study was to evaluate the clinical applicability of the segmental reconstruction of maxillary defects by arced distraction osteogenesis of zygoma with the self-invented distractor.Methods:The study was performed on resin model of human skull.First,most of the right maxilla was excised to make maxillary defect,then the bony transport disk 17 mm in width posterior to the defect was made and the internal arced and straight distractors were fixed.The imitation started with the transport disk moved forward along the orbit driven by the central bar.Results:The arced distractor was fit for the bony anatomy of zygoma and maxilla.The zygoma transport disk was distracted about 20 mm around an arc to be fixed in the expected position.Conclusion:In the case of large maxillary defects,the internal distraction osteogenesis of zygoma could be performed to set up bony support to restore the midface appearance and prepare for the later functional reconstruction.
