艾灸与药物穴位治疗机械通气并发腹胀的临床研究

目的探讨机械通气并发腹胀的治疗方法,为临床提供新的线索和途径。方法对2004年5月~2005年5月收治的90例机械通气并发腹胀患者,随机分为对照组(A组)20例、维生素B1组(B组)22例,吴茱萸组(C组)23例,综合治疗组(D组)25例。A组采取常规处理;B组采取常规处理、艾灸配合维生素B1封闭足三里;C组常规处理、艾灸足三里、吴茱萸外敷神阙穴;D组常规处理、艾灸配合维生素B1封闭足三里、吴茱萸外敷神阙穴。经A、B、C三组治疗无效的患者,采用综合治疗组方法进行治疗。结果D、B、C三组治疗有效率分别为100%、90.9%、91.3%,均显著高于A组55.0%(均P<0.01);D、B、C三组显效率分别为96.0%、68.2%、69.6%,也显著高于A组25.0%(均P<0.01);D组与B、C两组之间有效率比较无明显差异(P>0.05),但D组显效率显著高于B、C两组(P<0.01);A、B、C三组治疗无效的13例患者采用综合治疗组方法进行治疗,腹胀均获得缓解。结论艾灸配合维生素B1封闭足三里、吴茱萸外敷神阙穴方法治疗机械通气并发腹胀,其操作简单方便、实用性强、效果显著。

Clinical study on moxibustion and drug therapy by acupoint of abdominal distention with mechanical ventilation

Objective To study the method of treating abdominal distention with mechanical ventilation,and supply new clue and method for clinical treatment. Methods 90 cases patients who suffered from abdominal distention with mechanical ventilation were divided into the following groups: contrast group(group A) 20 cases,vitamin B1(group B)22 cases,Wu-zhu-yu group(group C) 23 cases,and synthetical treatment group(group D) 25 cases.Group A takes normal measure,Group B takes normal measure as well as moxibustion coordinate vitamin B1 closed Zu-san-li poid,group C takes normal measure,moxibustion Zu-san-li and Wu-zhu-yu external apply accupoin,group D take all the methods of A,B and C.13 cases who failed to respond to medical treatment by all of the three methods were cure by the method of group D.Results The effective rate in group D,B and C were 100%,90.9% and 91.3% respectively,and was obviously higher than that of group A,which was 55.0%(P<0.01).The display rate in group D,B and C was 96.0%,68.2% and 69.6% is obviously higher than that of group A,which was 25.0%(P<0.01).There was no significant difference in effective rate in group D,B and C (P>0.05),but display rate in group D was higher than in group B and C(P<0.01);13 cases who failed to respond to treatment by A,B and C got difference treatment by synthetical treatment.Conclusions Moxibustion coordinate vitamin B1 closed Zusan-li and Wu-zhu-yu external apply accupoin was a practical method on reliving abdominal distention with mechanical ventilation.
