| 摘 要： | 目的探讨在经后正中入路应用经关节螺钉联合侧块螺钉双向内固定的疗效。方法2005年1月至2008年10月对23例患者通过后路应用经关节螺钉联合侧块螺钉行颈椎后路双向内固定治疗,其中创伤性骨折脱位15例,颈椎间盘突出伴椎管狭窄8例。观察固定效果。结果共置经关节螺钉47枚,其中C4、56枚,C5、633枚,C6、78枚;共置侧块螺钉46枚,C28枚,C318枚,C416枚,C54枚。术中所有螺钉均成功置入,未出现椎动脉、神经根和脊髓损伤等并发症。23例患者均获随访,随访时间10个月~3年6个月,平均21个月,手术切口较传统术式减少1倍,植骨融合时间2~5个月,平均3.2个月。结论通过后路固定颈椎时,采用经关节螺钉联合侧块螺钉双向固定,固定简单、可靠,植骨融合率高,减少手术切口长度,降低了血管、神经根损伤风险,减少轴性症状等并发症,取得良好效果。
