摘    要:Objective To investigate factors affecting limb swelling after primary total knee arthroplasty and to guide and improve patient rehabilitation. Methods Using the hospital database, we retroactively examined the mean changes in limb circumferences of 286 consecutive patients who underwent primary total knee arthroplasty(TKA) between October 2007 and August 2009. The effect of age, gender, body mass index (BMI), presence of deep vein thrombosis (DVT), type of anti-coagulation, length of operation, hidden blood loss(HBL) and type of prosthesis on post-operative limb swelling was studied. Results Swelling usually affected both lower limbs, with swelling significantly more pronounced in the operated limb than the non-operated limb. The swelling above the knee is also significantly greater than swelling under the knee. The change in limb circumference at 10cm below the knee between patients with BMI≤25kg/m2 and patients with BMI>25kg/m2 was not found to be significantly different. However, the change in limb circumference at 10cm above the knee was found to be significantly different between the two groups. No statistically significant difference in limb swelling was detected between the different age groups (P>0.05). Similarly, gender, type of anti-coagulation, presence of DVT, and type of prosthesis all did not affect post-operative limb swelling. Length of operation was also not found to be correlated with limb swelling. Multivariate linear regression analysis shows that, the impact factors of postoperative limb swelling include body mass index (BMI) and hidden blood loss(HBL). Conclusion ①Limb swelling after TKA is most pronounced during post-operative days 3 to 5, and is related to early post-operative hidden blood loss. The patient’s hemoglobin level should be monitored. ② There exists a correlation between the degree of limb swelling and the patient’s BMI and hidden blood loss(HBL). ③ DVT in intramuscular venous plexuses after TKA have little effect on limb swelling.
