老年肺部感染189例临床分析

目的探讨老年人肺部感染的临床特点，以便指导诊治。方法回顾性分析了189例老年肺部感染患者，并与同期216例青壮年患者进行对照。结果老年肺部感染发热者占59.79%，青壮年占93.98%(P＜0.01)，早期无呼吸道症状者老年组占41.26%，对照组为30.09%(P＜0.05)；老年肺部感染呼吸衰竭的发生率为38.10%，对照组为0.29%(P＜0.01)；老年组同时患有其他疾病者占肺部感染总人数的91.53%，青壮年组为56.48%(P＜0.01)；老年组肺部感染中院内感染占19.57%，对照组为3.70%(P＜0.01)；两次以上感染老年组为14.28%，对照组为1.39%(P＜0.01)；病死率老年组为29.10%，青壮年组为1.39%(P＜0.01)。结论老年人肺部感染症状不典型多，易发生呼吸衰竭，伴随疾病及并发症多，院内感染及两次以上感染者多，病死率高。

Analysis of Elderly Pulmonary Infection in 189 Cases

Objective To investigate the clinical characteristic of pulmonary infection in the elderly patients,so that instruct to make a diagnosis and give treatment.Methods Look back to analyze 189 cases of pulmonary infection in the elderly patients in contrast with 216 cases of adult group.Results The percentage of elderly with a fever was 59.79% contrasted with the adult percentage of 93.98%(P<0.01),nonspecific symptoms in the early stage in the elderly group was 41.26% contrasted with the adult of 39.09%(P<0.05),repsiratory failure combined with the elderly was 38.01% contrasted with the adult of 0.29%(P<0.01),accompanied other disease of the elderly was 19.57% and it occurred in the adult was 3.70%(P<0.01),over two times infections occurred in the elderly was 14.28% with the adult of 1.39%(P<0.01),mortality among the elderly group was 29.10% contrasted with the 1.39% among thd adult (P<0.01).Conclusions These dangerous factors of subclinical symptoms,respiratory failure,accompanied diseases,complications,hospital acquired infections,over two times infections and mortality of pulmonary infections in the elderly group are very higher than that in the adult group.
