晚期胃癌的手术治疗

目的:为晚期胃癌的合理治疗提供依据。方法:将1990年至2007年256例IIIb、IV期胃癌患者按其接受不同的手术方式分为姑息性切除组164例和非切除组92例,对比二组间手术效果。结果:姑息切除手术并发症大于非切除组(P<0.05),手术死亡二组差别不大(P>0.05),2年生存率及术后生存质量在姑息切除后能得到明显改善(P<0.05)。结论:姑息切除术效果优于非切除术,晚期胃癌应尽可能争取行姑息切除病灶。

Operation for Advanced Gastric Carcinoma

Objective:To provide basis for reasonable treatment for patients with advanced gastric carcinoma.Method:In accordance with different operation 256 patients of TNM stage IIIb and IV gastric carcinoma from 1990 to 2007 were divided into palliative resection group(n=164) and non-resection group(n=92).The result of operation were compared between two group.Result: The resection group occurred more postoperative complications than the non-resection group(P<0.05).No significant difference in operative mortality(P>0.05).The two-year survival rate and quality of life in resection group was higher than non-resection(P<0.05).Conclusion:The effect of palliative resection is better than that of non-resection.It is preferable that palliative resection be performed for the patients with advanced gastric carcinoma.
