基于DRGs的某医院新生儿科医疗业务评价

目的　分析某医院新生儿科出院病人DRGs分组情况，评价医疗组住院服务指标，为DRGs病组优化、医院精细化管理提供依据。方法　以2015-2016年新生儿科7189例出院患者作为研究对象，对病案数据和DRGs分组资料进行收集分析。结果　出生天数为1~5天的患者占总研究对象的69.37%，低出生体重儿约占33.58%；对纳入病例进行DRGs分组，足月产（体重>2499g）伴严重问题（PU11）组入组患者最多，约占46.15%，其次是早产儿（体重1500~2499g)伴严重问题（PT11）组，约占21.34%；对医疗组进行DRGs指标分析，A、B组入组例数较多，病种覆盖面广，总权重较大，C组CMI值最高，两年分别为3.30和3.29。结论　该院新生儿科收治患者较严重，医疗技术难度较大，医疗组收治情况各异，引入DRGs指标可以客观公平的评价科室工作强度、医疗难度以及医疗消耗等情况。

Evaluation of neonatal medical services in a hospital based on DRGs

Objective　To evaluate the medical service quality for the neonatal department with the help of DRGs, and to provide references for continuous improvement for DRGs optimization and refined hospital management. [WTHZ]Methods　A total of 7189 patients from the neonatal department from 2015 to 2016 were taken as a subject, and the medical record data and DRGs data was collected and analyzed. [WTHZ]Results　The number of patients born with 1-5 days accounted for 69.37% of the total research objects, and the low birth weight children accounted for about 33.58%. The patients in the PU11 group were the most, accounting for 46.15%, followed by the PT11 group, accounting for 21.34%. Analyzed the DRGs index of medical groups, the number of patients in the group A and B were larger, and group C had the highest case mix index (CMI) value, with 3.30 and 3.29 in two years, respectively. [WTHZ]Conclusion　The patients from neonatal department suffer from more serious diseases, and the medical technology is difficult. The treatment among medical group have different situations. The introduction of DRGs indicators can help evaluate the work intensity, medical difficulty and medical consumption objectively and fairly.
