腹部彩超与MRCP对胆总管结石诊断价值的比较

目的:比较腹部彩超与MRCP对胆总管结石的诊断价值。方法:对79例做过腹部彩超及MRCP两项检查的胆总管结石的患者,按照结石在胆总管的不同位置分为胆总管上段(十二指肠上段)、胆总管中段(十二指肠后段)、胆总管下段(即胰腺段和壶腹段),将检查结果与术后结果进行对照。结果:超声与MRCP对胆总管中上段结石的诊断率,差异无统计学意义(P>0.05),对胆总管下段结石MRCP诊断准确性略优于超声。结论:腹部彩超对胆总管中上段结石定位、定性诊断的准确率较高,具有很高的临床应用价值,应作为首选胆总管结石诊断的常规检查,同时对胆总管下段结石与MRCP联合应用,将极大提高诊断的准确率。

Comparison of the diagnostic value of abdominal color ultrasound and MRCP in choledocholithiasis

Objective To evaluate the diagnostic value of abdominal color ultrasound and MRCP in choledocholithiasis.Method 79 choledocholithiasis patients checked by abdominal color ultrasound and MRCP were divided into three groups： the up-choledoch（up-duodenum）,middle-choledoch（behind-duodenum） and the distal choledoch（pancreas segment and ampullae part）,then we compared the examination results of color ultrasound and MRCP with the operation results in accordance with the different parts of choledocholithiasis.Results Abdominal color ultrasound and MRCP were not significantly different in the upper-middle choledocholithiasis（P 0.05）,but in distal choledocholithiasis,MRCP was slightly better than the color ultrasound.Conclusion Abdominal color ultrasound has higher accuracy to quantitative,qualitative diagnosis of the upper-middle choledocholithiasis and has a high value of clinical application value,which should be chosen as the first routine examination;combining color ultrasound and MRCP can greatly raise the diagnostic accuracy to distal choledocholithiasis.
