腹腔镜在经阴道非脱垂子宫切除术中的应用价值

摘    要:目的:探讨腹腔镜在经阴道非脱垂子宫切除术(LAVH)中的临床应用价值。方法:将本院2007年2月-2010年10月间78例需全子宫切除术患者分为腹腔镜辅助阴式子宫切除组(LAVH组,40例)和经阴道子宫切除术组(TVH,38例),观察两组手术时间、术中出血量、术后肛门排气时间及住院天数。结果:与TVH组比较,LAVH组术中出血量少、手术时间短,差异均有统计学意义(P〈0.05),两组术后并发症、术后排气时间、术后发热率比较差异均无统计学意义(P〉0.05)。结论:LAVH是较为理想的子宫切除术方式,较单纯经阴道子宫切除具有易掌握、创伤小、并发症少等优点,尤其适用于既往有盆腔手术史、暴露困难的患者和阴式手术经验相对少的医师。

The Value of Laparoscope to the Transvaginal Hysterectomy for the Non-prolapsed Uterus

Abstract:Objective:To explore the feasibility of laparoscope to the transvaginal hysterectomy for the non-prolapsed uterus.Methods:78 cases under transvaginal hysterectomy with or without laparoscopic help in our hospital from Feb 2007 to Oct 2010 were analyzed retrospectively.40cases were under laparoscopic assisted transvaginal hysterectomy(LAVH) , and 38 cases were under total transvaginal hysterectomy(TVH).The duration of operation, amount of bleeding during operation, postoperative anal exhaust time, highest body temperature, hospitalization time, and pain were compared between the two groups.Results:Both The duration of operation and the amount of bleeding during operation in LAVH group are statistically lower than those in TVH group.No statistiacally differences were found in postoperative anal exhaust time, highest body temperature and hospitalization time between two groups.Conclusion:With the characteristics of easy grasping, minimal Invasion, and lower complication, LAVH is a relatively good one for hysterectomy , especially appliable for the patients with pelvic surgery history , or hard to expose incision and for the doctors with less experiences of transvaginal surgery.
