三维适形超分割放疗治疗晚期食道鳞癌的疗效分析

目的 探讨三维适形超分割放疗对晚期食道鳞癌的临床疗效.方法 回顾性分析笔者所在医院2009 年1 月-2011 年4 月收治的85 例晚期食道鳞癌患者,采取CT 模拟定位技术和三维适形超分割放疗技术进行治疗,1.2 Gy/ 次,2 次/d,每次间隔时间大于6 h,5 d/ 周,总剂量48~60 Gy,中位剂量52 Gy.放疗结束后评价近期疗效：临床症状变化及3 个月后CT 片、食管钡餐片.结果 所有患者放疗后随访3 个月~3 年,1、2、3年局部控制率分别为86%、52%、41%;1、2、3年生存率分别为92%、65%、57%;放疗后5 例患者完全缓解,77 例患者部分缓解,3 例患者无改变,总体有效率为96.5%;40 例患者出现急性放射性食管炎;40 例患者出现白细胞下降情况.结论 三维适形超分割放疗治疗晚期食道癌取得了良好疗效,有效地改善了患者临床症状,延长了生存期,提高了患者生存质量,值得临床上广泛的推广.

Curative Effect Analysis on 3-dimensional Conformal Radiotherapy for Esophageal Cancer

Objective ：To explore the efficacy of 3-dimensional conformal （3DCRT） in management of esophageal carcinoma.Methods ：Clinical 85 patients with esophageal carcinoma were treadef by 3DCRT, 1.2 Gy a time, 2 times a day, 5 day a week , every time interval time more than 6 h 48-60 Gy in total, tmedian does 52 Gy.Short-term curative effect evaluation affter radiotherapy ： clinical symptoms change, CT, EBME after 3 mouths. Results ：All of the patients after radiotherapy were follow-up of 3 months to 3 years, complete relief was observed in 22 cases.partial relief in 7 cases and noresponse in 2 cases.The overall response rate reached 96.5% ; the 1, 2 and 3-year survival rates were 92%, 65% and 57% ; the 1, 2 and 3-year local control rates were 86%, 52% and 41%, respectively ; complete relief was observed in 22 cases.partial relief in 7 cases and noresponse in 2 cases ; The overall response rate reached 93%.Acuter adiations esoph-agitis occurred in 40 of the cases ; 40 cases observed leucopenia.Conclusion ：3DCRT is an effective for treatment of patients with esophageal cancer, which can improve the clinical symptoms, extende the patient survival, improve the patient survival quality, it is worth clinically promotion.
