| 摘 要： | 目的:探讨无创正压机械通气(NIPPV)在老年慢性阻塞性肺疾病(COPD)合并重症呼吸衰竭患者中的应用效果与护理方法.方法:对35例患者采用配备双水平气道正压通气的无创呼吸机经面罩进行机械通气治疗,并给予精心护理.结果:本组31例好转出院,4例死亡,病死率达11.43%.NIPPV治疗后pH、PaO2、SaO2较治疗前升高(P＜0.05,P＜0.01),PaCO2较治疗前降低(P＜0.01);NIPPV治疗后患者RR、HR、APACHEⅡ、GCS、辅助呼吸肌评分与治疗前比较差异均有统计学意义(P＜0.05,P＜0.01).结论:NIPPV治疗COPD合并重症呼吸衰竭创伤小、操作简便,值得临床推广应用.
