影响急性脑出血患者血液流变学的因素分析

目的探讨脑出血患者血液流变学改变的影响因素,以指导临床采取及时、正确的干预措施,促进神经功能的恢复。方法收集神经内科住院的脑出血病例157例,均经头颅CT确诊,测定其血液流变学指标,并对可能影响脑出血患者血液流变学指标的因素(高血压病史、糖尿病史、高脂血症、冠心病史、出血量、出血部位、血肿是否破入脑室、入院时血压、神经功能缺损评分、入院后进食情况、脱水剂、体温和预后)进行单因素分析,把有意义的变量引入两分类反应的非条件Logistic回归分析。结果采用两分类反应变量的非条件Logistic回归分析,最后进入Logistic回归模型的影响高切血粘度的因素有6个,分别是高血压病史、糖尿病史、冠心病史、出血量、入院后进食情况和脱水剂。结论引起ICH患者血粘度增高的主要因素有高血压病史、糖尿病史、冠心病史、出血量、脱水剂及入院后进食情况。

The risk factors of hemorrheology in patients with acute intracerebral hemorrhage

Objective To determine the risk factors of hemorrheology in patients with intracerebral hemorrhage(ICH).Methods The hemorrheological parameters in patients with ICH were determined.The possible risk factors of hemorrheology(hypertensive disease,diabetes,hyperlipemia,coronary artery disease,hematoma volume,bleeding part,hematoma invading cerebral ventricle,blood pressure,diet state,temperature,dehydrant and prognosis) were probed.The related risk factors were analyzed by binary logistic regression method.Results There were six risk factors influencing whole blood viscosity in high shear,which were hypertensive disease,diabetes,coronary artery disease,hematoma volume,diet state and dehydrant.Conclusion The main risk factors inducing hyperviscosity are hypertensive disease,diabetes,coronary artery disease,hematoma volume,diet state and dehydrant.
