高频彩色多普勒超声在甲状腺癌诊断中的应用价值

目的分析及总结甲状腺癌的彩色多普勒超声声像图特征，探讨高频彩超对甲状腺癌的诊断价值。方法对25例经手术及病理确诊为甲状腺癌的高频彩超声像图资料进行回顾性分析。结果16例表现为不均质低回声，5例表现为强弱不均质的混合性回声，4例表现为强回声；单发性结节15例，多发性结节10例；癌结节仅出现在左或右一侧腺体者共18例，两侧腺体均有者7例；癌结节的边界多不规整，血流信号多较丰富；12例结节内发现砂粒样钙化。结论高频彩超对甲状腺癌具有较大的诊断价值，但有时易与某些良性甲状腺肿块相混淆，故把二维及彩色多普勒等多项超声指标结合起来进行判断将有助于提高对甲状腺癌的诊断准确率。

Value of High Frequency Color Doppler Ultrasound in the Diagnosis of Thyroid Carcinoma

Objective To evaluate the value of high frequency color Doppler ultrasound in the diagnosis of thyroid carcinoma by analyzing and summing the color Doppler ultrasonic features of thyroid carenoma. Methods The high frequency color Doppler ultrasonic features of thyroid carcinoma in 25 cases were analyzed retrospectively after they were resected by operation. Results Of the 25 cases of thyroid carcinoma, 16 cases appeared uneven hypoechoic, 5 cases mixed echoes while another 4 cases hyperechoic;there were 15 single nodules and 10 multinodules respectively ;the carcinoma nodules were only in one side of the thyroid in 18 cases while both the two sides of the thyroid gland were found carcinoma nodule in the emailing 7 cases. Aside with the micro calcification in 12 cases,most of the nodules had irregular boundaries and abundant blood flow signals. Conclusion As for thyroid carcnoma, high frequency color Doppler ultrasound is of moderate diagnostic value,and both the grey-scale uhrasonography and the color Doppler blood flow imaging should be combined to improve the ultrasonic diagnosis rate since they may be mistaken for benign ones.
