三种术式切除非脱垂子宫的临床效果比较

目的：比较腹腔镜辅助阴式子宫切除术（LAVH） 与阴式子宫切除术（TVH） 及经腹全子宫切除术（TAH） 的手术效果.方法：回顾性分析146 例全子宫切除术患者的临床资料,比较手术时间、出血量、住院时间、术后镇痛率、肛门恢复排气时间及并发症.结果：LAVH 组的手术时间比TVH、TAH 组明显延长（P〈0.05）,LAVH、TVH 组术后住院日数、镇痛率、肛门恢复排气时间明显少于TAH 组（P〈0.05）,而三组患者在术中出血量、手术并发症等方面差异无统计学意义（P〉0.05）.结论：微创手术较腹式手术具有创伤小、成功率高、住院时间短、镇痛率低、恢复快等优点,且LAVH 能扩大TVH 的适应证,值得推广.

Comparison of Clinical Effects of Three Surgical Resection of the Non-prolapsed Uterus

Objective：To compare the effect of laparospic assisted vaginal hysterectomy surgery（LAVH）, vaginal hystereetomy surgery（TVH） and resection of abdominal hysterectomy （TAH）.Method ：A retrospective analysis of 146eases of hysterectomy was eonducted on clinical data, operative time, bleeding, length of stay, postoperative analgesia rate anus to restore the exhaust time and complications. Result ：Comparing the operative time, LAVH group was much longer than TVH and TAH group（P〈O.05）, while for LAVH, TVH group, postoperative length of stay, analgesic rate anus to restore the exhaust time is significantly less than the TAH group（P〈0.05）, but there is no significant difference in operation blood loss, surgical complications among the three groups.（P〉0.05）.Conclusion ：Minimally invasive surgery is less invasive than abdominal surgery, accompanied with a high success rate, shorter length of stay, low analgesia rate, quick recovery, expanding the TVH indications, therefore it is worth promoting.
