摘    要:目的:研究原发性高血压 (EH) 患者动态血压监测指标和左室肥厚 (LVH) 的关系,为临床决策和治疗提供依据.方法:收集宁夏医科大学总医院心内科患者 90 例,行超声心动图和 24 h 动态血压监测 (ABPM),根据左室重量指数 (LVMI) 分为 EH 伴 LVH 组 (n=37)、EH不伴 LVH 组 (n=23) 和对照组 (n=30),比较组间差异,分析动态血压各参数和 LVMI 的相关性.结果:(1) 收缩压、夜间舒张压、收缩压负荷和LVMI,EH 伴 LVH 组 〉EH 不伴 LVH 组 〉 对照组 ;(2)LVMI 与收缩压、舒张压、脉压、血压变异性和夜间血压负荷呈正相关,与夜间血压下降率呈负相关.结论:左室肥厚与动态血压水平、脉压、血压变异性和夜间血压负荷呈正相关,与夜间血压下降率呈负相关.

The Correlation Studies between Ambulatory Blood Pressure Monitoring Parameters and Left Ventricular Hypertrophy in Essential Hypertension

Abstract:Objective :To study the relationship in the Essential Hypertension (EH) patients of ambulatory blood pressure monitoring index with left ventricular hypertrophy (LVH) , and provide a basis for clinical decision and treatment.Method :Collecting Ningxia Medical University hospital affiliated 90 patients with heart medicine, llne eehocardiography and 24 hours ambulatory blood pressure monitoring (ABPM), according to the left ventrieular weight index (LVMI) was divided into EH with LVH group (n = 37), EH withnot LVH group (u=23) and the control group (n=30), compare the difference between groups, analyze the dynamic blood pressure parameters and LVMI relevance. Result :(1) Systolic blood pressure, night diastolic blood pressure, systolic blood pressure load and LVMI, EH with LVH group was higher than EH without LVH group, EH without LVH group was higher than those in the control group; (2) The correlation between LVMI and systolic blood pressure, diastolic blood pressure, pulse pressure, blood pressure variation, nighttime blood pressure load were positive association, between LVMI and nighttime blood pressure decrease rate was negative correlation.Conclusion :Left ventrieular hypertrophy and dynamic blood pressure level, pulse pressure, blood pressure variability and blood pressure load in the night are positively associated, between LVMI and nighttime blood pressure decrease rate is negative correlation.
