对1例原发性高血压合并冠心病患者的药历分析与药学监护

目的：探讨药师在原发性高血压合并冠心病患者的治疗中如何进行药学监护,保证用药的合理性和安全性。方法：通过分析1例原发性高血压合并冠心病患者的临床治疗过程,为患者提供全方位的药学服务,重点监护药品的不良反应。结果：药师对药历进行分析和提供药学监护,降低了药品不良反应的发生率,提高了医疗质量。结论：药师个体化、全方位的用药服务,在提高医疗质量、预防潜在的危险、构建和谐的医患关系方面发挥了重要的作用。

Pharmaceutical Care for 1 Case of Patient with Hypertension Associated with Coronary Heart Disease

OBJECTIVE：To discuss the approaches for pharmacist to carry out pharmaceutical care for patients with hypertension associated with coronary heart disease （CHD） in an attempt to ensure the rationality and safety of drug use. METHODS： Through analysis of clinical diagnosis and treatment for 1 case of patient with hypertension associated with coronary heart disease for this case an omni-bearing pharmaceutical care was provided with focus on monitoring of adverse drug reaction （ADR）. RESULTS： Pharmacist＇ analysis on medication history and the pharmaceutical care provided by the pharmacist resulted in lower incidence of ADR and improved quality of medical care. CONCLUSION： The individualized omnibearing pharmaceutical care provided by pharmacist contributed to improvement of the quality of medical care, prevention of the potential risk and construction of harmonious doctor-patient relationship.
