CT、病理和CK19 RT-PCR在诊断肺癌淋巴结转移的对照研究

摘    要:目的 :探索检测肺癌淋巴结转移的新途径。方法 :应用CT、病理和细胞角蛋白 19(CK19)逆转录聚合酶链反应 (reverse transcriptasepolymerasechainreaction ,RT PCR)法 ,对 15例可手术的原发性肺癌患者 60枚区域淋巴结分别进行癌转移检测。结果 :60枚区域淋巴结中 ,16枚经CT诊断为转移 ,其中有 11枚CK19RT PCR为阳性 ,4 4枚CT阴性中有 2 4枚阳性 ;15枚经病理诊断为转移的 ,CK19RT PCR均为阳性 ,而 4 5枚病理诊断为阴性的淋巴结中有 2 0枚为阳性。结论 :与传统的CT、组织病理检查相比 ,CK19RT PCR法可提高淋巴结癌转移的检出率 ,从而准确地评价其转移状况

Abstract:Objective To set up a new method to detect metastasis of regional lymph nodes in patients with lung cancer.Methods 60 lymph nodes obtained from 15 patients with lung cancer were analyzed by CT,pathology and CK19 RT PCR.Results CK19 mRNA was found in 11 of 16 lymph nodes proved to be metastasized by CT scan, of the 44 metastasis negative lymph nodes ,24 were found to express CK19 mRNA; CK19 mRNA were detected in all of the 15 lymph nodes which were histologically metastasis positive, of the 45 lymph nodes which were metastasis negative,20 were found to express CK19 mRNA.Conclusions Comparing with CT and pathology ,the CK19 RT PCR can make more accurate assessment of metastastic status in lymph nodes.
