血清瘦素及炎性细胞因子改变在妊娠高血压疾病中的诊断价值

目的:探讨血清瘦素(Leptin)、TNF-α、IL-6、IL-8、IL-10在妊娠期高血压疾病患者中的变化及其诊断价值。方法:采用酶联免疫吸附分析法测定实验组60例妊娠高血压疾病患者(妊娠期高血压20例,轻度子痫前期20例,重度子痫前期20例)和正常晚期妊娠孕妇(对照组)35例血清Leptin、TNF-α、IL-6、IL-8、IL-10水平并进行比较,绘制ROC曲线观察其对妊娠期高血压疾病的诊断价值。结果:实验组患者血清Leptin、TNF-α、IL-6、IL-8水平明显高于对照组,IL-10水平明显低于对照组;同时,轻度子痫前期、重度子痫前期患者的血清Leptin、TNF-α、IL-6水平明显高于妊娠期高血压患者;重度子痫前期患者的血清TNF-α、IL-6水平明显高于轻度子痫前期患者;ROC曲线分析血清Leptin与TNF-α的诊断特性最好,IL-8的诊断特性最低。结论:妊娠期高血压疾病患者血清Leptin和炎性细胞因子均发生改变,并在一定程度上反应病情的轻重,可做为联合诊断和病情评估的重要指标。

Changes and diagnosis values of serum leptin,TNF-,IL-6,IL-8 and IL-10 in hypertensive disorder complicating pregnancy women

Objective:To investigate the changes and diagnosis values of serum leptin,TNF-α,IL-6,IL-8 and IL-10 in patients of hypertensive disorder complicating pregnancy(HDCP).Metholds:Enzyme-linked immunoabsorbant assay(ELISA)was used to measure the serum level of leptin,TNF-α,IL-6,IL-8 and IL-10 in 60 HDCP women(including 20 with gestational hypertension,20 mild pre-eclampsia,and 20 severe pre-eclampsia)and 35 normal pregnant women.ROC curves were analyzed to all datas.Results Serum levels of leptin,TNF-a,IL-6,IL-8 in HDCP women were increased significantly when compared with normal pregnant women and IL-10 levels was deceased significantly at the same time;Serum levels of leptin,TNF-a,IL-6of mild pre-eclampsia and severe pre-eclampsia women were higher than those of gestational hypertension women.Serum levels of TNF-a and IL-6 of severe pre-eclampsia were higher than those of mild pre-eclampsia.ROC curves showed that leptin and TNF-α had the best diagnosis sensitivity and specificity.Conclusions In HDCP women,serum leptin and inflammatory cytokines which can be uesd to diagnosis and evaluate the pathogenetic condition were changed greatly and indicated the prognosis.
