Hp阳性十二指肠溃疡组织中NOS的表达

摘    要:目的 研究一氧化氮合成酶 (NOS,包括 n- c NOS,e-c NOS)在 H p阳性的十二指肠溃疡 (DU)组织中的表达及治疗前后的变化 ;探讨 DU的发生与 H p感染的关系并对其疗效、预后提供参考依据 .方法 采用免疫组织化学方法对 6 0例 DU胃镜活检标本观察其 NOS的表达 ,包括 H p阳性 30例 ,H p阴性 19例 ,DU治疗前 H p阴性经治疗后仍为阴性 3例 ,DU治疗前 H p阳性经治疗后转为阴性 8例 .结果  H p阳性的 DU组织中 NOS的阳性反应强度明显高于 H p阴性的 DU组织 [n- c NOS:P1 <0 .0 1vs H p(- ) .e- c NOS:P2 <0 .0 1vs H p(- ) ],治疗后 H p感染转阴者 NOS的表达有不同程度的减低 (n- c NOS:P1 <0 .0 1vs治疗前 ,e- c NOS:P2 <0 .0 5 vs治疗前 ) ,而治疗后 H p感染仍为阳性者 ,则无差异 .结论 在 H p引起 DU的发病机制中 ,NOS可能是其致病因素之一 ,根除 H p可使 DU组织中 NOS降低 ,NOS是否可作为一项 DU疗效及预后的指标 ,值得进一步探讨

Abstract:AIM To study the relationship between nitric oxids synthase (NOS) expression level and Helicobacter pylori (Hp) infection in patients with duodena1 ulcer (DU). METHODS NOS expressions were studied by immunohistochemical method. 60 cases of DU were divided into 4 groups: 30 cases of Hp positive patients (group A), 19 cases of Hp negative patients (group B), 3 cases of Hp positive patients who were proved still Hp positive after treatment (group C), 8 case of Hp positive patients who were proved Hp negative after treatment (group D). RESULTS NOS expression levels in Hp positive patients were significantly higher than those in Hp negative patients [n cNOS: P 1< 0.01 vs Hp (-) . e cNOS: P 2<0.01 vs Hp (-)]. In group C, the NOS expression level had no changes after treatment. In group D, the NOS expression decreased at different levels after treatment (n cNOS: P 1<0.01 vs before treatment, e cNOS: P 2<0.05 vs before treatment). CONCLUSION NOS may play a role in the mechanism of DU caused by Hp infection. It is worthy of further study as to whether NOS can be used as some index in the dignosis and prognosis of DU.
