川芎嗪治疗急性脑血栓形成病人的甲皱微循环变化

摘    要:<正> 川芎嗪(四甲吡嗪 Tetramethyl Pyra-zine)系川芎中的一种有效生物碱,药理研究证明它具有改善微循环的作用。临床上使用川芎嗪治疗急性缺血性脑血管病有肯定的疗效。至今尚未见到急性脑血栓形成的病人使用川芎嗪治疗后甲皱微循环的观察报告。为了观察川芎嗪治疗急性脑血栓形成病人甲皱微循环的变化,对比偏瘫病人健、瘫侧甲皱微循环的变化以及探讨急性脑血栓形成病人甲皱微循环检查的临床意义,我们观察了28例川芎嗪治疗的急性脑血栓形成病人的双侧甲皱微循环,其结果如下:


CHANGES OF THE NAILFOLD MICROCIRCULATION OF THE PATIENTS WITH ACUTE CEREBRAL THROMBOSIS TREATED BY LIGUSTRAZINE
Abstract:The results of Ligustrazine effects on the nailfold microcirculation in 28 cases of acute cerebral thrombosis with hemiplegia were reported.The index- es in the nailfold microcirculation were the number and length of the capil- lary loops,the degree of the microvascular permeability,the blood flow condition and the skin temperature in the ventral aspect of the fourth finger. The twenty eight healthy people were studied as a control group.The bilateral nailfold microcirculation was compared each other at all observations. Prior to the treatment with Ligustrazine,it was found that:(1)among the 28 cases,there was an increase in permeability of 17 cases (60.7%) in the sound side and 22 cases (78.9%) in the affected side and the degree of permebility was espicially severe in the affected side;(2)the number of the capillary loops decreased in the affected side of 10 cases;(3)the length of the capillary loops shortened in the affected side of 11 cases (4)the abnormal condition of blood flow was seen in both sides about two-thirds of the patients;(5) the mean of the skin temperature of fourth finger in the paralyzed side was lower than that of the opposite side by 1.41℃.After treatmeut the increased permeability decreased singnificantly in the affected side,the number of the capillary loops of 7 cases and the length of the capillary loops of 7 cases resumed to normal range and the other indexes varied without statistical significance. According to the results of the observations,we may conclude that in the patients with acute cerebral thrombosis,an increase in permeability is a feature of the nailfold microcirculation,and the degree of the permeability may be used as an auxiliary diagnosis and as a sign of dynamic observations during the treatment with Ligustrazine. In the present article we suggest a trial criterion for classification of permeability in the nailfold microcirculation for reference.
