拔牙断根误入上颌窦的鼻内窥镜取出方法研究

目的阐述拔牙过程中断根进入上颌窦时使用鼻内窥镜取出的手术方法。方法对在拔牙过程中断根进入上颌窦的9例患者中的10个牙根经上颌窦前壁开窗鼻内窥镜直视下取根。结果9例患者中的8例均快速取出断根，其中1例延期取根的患者在取出断根的同时接受了上颌窦根治术；1例患者由于断根位于窦黏膜下未进入窦腔而未取，长期观察无不良反应。所有患者经长期观察均无继发上颌窦炎及上颌窦瘘的发生。结论采用经上颌窦前壁开窗鼻内窥镜直视下取根法可以方便快捷地取出进入上颌窦的断根，手术时间短、对患者的创伤小，是一种非常有效的方法。

Clinical study on technique of endoscopic removal of broken teeth roots from maxillary sinus

Objective To study the technique about endoscopic removal of broken teeth roots dislocated into the maxillary sinus in the surgery of teeth extraction.Methods Ten broken teeth roots of nine patients dislocated into maxillary sinus in the surgery of teeth extraction were removed by endoscope through windowing in anterior wall of the maxillary sinus.Results Eight out of nine patients were removed rapidly, another one patient underwent the surgery of radical maxillary antrotomy in the meantime; one patient's broken root was not removed since the root was located in the layer between the bone and the mucosa of sinuses. Neither maxillary sinusitis nor fistula of maxillary sinus was found in all of patients in review.Conclusion The technique about endoscopic removal of broken teeth roots through windowing in anterior wall of maxillary sinus is an efficient method, which has less wounds and shorter surgical time.
