放置IUD后宫腔微环境的研究

目的 :研究长期放置IUD子宫局部免疫功能的变化。方法 :以放置 5～ 2 0年惰性和含铜宫内节育器 (IUD)共 6 2例为研究组 ,以正常妇女 2 7例为对照组 ,以肿瘤坏死因子 α(TNF α)、白细胞介素 2 (IL 2 )、分泌型IgA(SIgA)和T淋巴细胞为指标。结果 :宫腔冲洗液TNF α浓度在惰性IUD组、含铜IUD组与对照组之间均未见明显差异。含铜IUD组宫腔冲洗液IL 2浓度显著低于对照组 (P <0 0 5 ) ,SIgA浓度亦显著低于对照组 (P <0 0 5 )。含铜IUD组子宫内膜组织抑制或杀伤T淋巴细胞 (CD+8淋巴细胞 )明显多于惰性IUD组和对照组 ;而辅助性T淋巴细胞 (CD4+ 淋巴细胞 )少于对照组 ,与惰性IUD组差异无显著性。结论 :长期放置IUD对子宫局部免疫功能有不同程度的影响

Study of influences of using IUD on intrauterine immune microenvironment

Objective:To investigate the influences of using intrauterine devices (IUD) for long term on intrauterine immune function.Methods:32 cases using inert IUD and 30 cases using a copper releasing IUD for 5 20 years were as study group,normal 27 cases as control group.Radioimmunoassay(RIA) was used to examine the concentration of tumor necrosis factor α(TNF α),interleukin 2 (IL 2) and secretory IgA(SIgA).Immunohistochemical technique was employed to demonstrate the population of T lymphocytes.Results: There was no significant difference in intrauterine fluid TNF α levels among the three groups.Intrauterine IL 2 levels of copper releasing IUD group was significantly lower than that in control group (P<0 05);Intrauterine SIgA level of copper releasing IUD group was significantly lower than that in control group (P<0 05).In endometrium of women using copper releasing IUD group,CD 8 + T lymphocytes were significantly more than those in control group and using inert IUD group,CD 4 + T lymphocytes were less than those in control group. Conclusion:There is some effect on intrauterine immune function of using IUD for long term.
