彩色多普勒血流显像对甲状腺肿瘤的诊断价值

目的:探讨彩色多普勒血流显像(CDFI)对甲状腺肿瘤的诊断价值。方法:对经手术病理证实的45例甲状腺癌和130例甲状腺腺瘤的二维及彩色多普勒超声进行比较分析。结果:甲状腺癌和甲状腺腺瘤在二维声像图、彩色血流丰富程度(P<0.01),肿瘤内部血管阻力指数(RI)、及收缩期峰值流速(Vmax)(P<0.05)方面具有显著性差异,钙化及低回声在甲状腺癌诊断的特异性较高,分别为95.3%和85.9%,甲状腺癌的超声诊断符合率为77.7%。结论:高频声像图中的钙化及低回声是诊断甲状腺癌的重要指标,而彩色血流丰富程度及频谱多普勒中的多项指标的变化将有助于提高甲状腺癌的诊断准确性。

The diagnostic value of color Doppler flow imaging in thyroid neoplasms

Objective: To study the diagnostic value of color Doppler flow imaging in thyroid neoplasms. Methods: Forty-five cases with thyroid carcinoma and 130 cases with thyroid adenoma, confirmed by pathology, were analyzed comparatively with two-dimensional and color Doppler ultrasound. Result: Significant difference was found between two diseases on two-dimensional imaging, more plentiful degree of color flow(P<0.01), higher resistance index(RI) and high Vmax(P<0.05) were observed in carcinoma group than those of adenoma group. Calcification and low echoes were specific signs for carcinoma. The incidence rate was 95.3% and 85.9% respectively. The accurate diagnostic rate of ultrasound was 77.7% for thyroid carcinoma. Conclusion: Calcification and low echoes in high frequency ultrasonography are important diagnostic indicators for thyroid carcinoma. The degree of color flow, higher Vmax or RI levels are helpful to improve the ultrasonic diagnostic accuracy of thyroid carcinoma.
