摘    要:目的了解近5年来我院临床分离大肠埃希菌的耐药变迁。方法采用纸片扩散法进行药敏试验,根据美国临床实验室标准化研究所(CLSI)文件的标准判断结果;采用WHONET-5软件对耐药性数据进行分析。结果 2004~2008年我院收集大肠埃希菌1932株,来自呼吸道标本、分泌物、脓液、尿液者分别占28.5%、19.9%、18.7%、17.3%。产超广谱β-内酰胺酶(ESBLs)大肠埃希菌检出率在43.6%~60.1%范围内。未发现对亚胺培南、美罗培南耐药的大肠埃希菌,产ESBLs大肠埃希菌多重耐药严重,临床常用抗菌药物的耐药性呈逐年上升趋势。结论加强大肠埃希菌细菌耐药监测,了解其耐药变迁,指导临床合理用药,防止耐药菌株的传播。

Abstract:Objective Objective To investigate the antimicrobial resistance of Escherichia coil isolated in the First Clinical Hospital of Jilin University during the past 5 years.Methods Disk diffusion test was used to study the antimicrobial resistance.The results were evaluated based on Clinical and Laboratory Standards Institute.WHONET 5 was applied for analysis.Results Most of 1932 strains of Escherichia coil were collected from respiratory secretion specimen(28.5%),secretions(19.9%),abscesses(18.7%) and urine(17.3%)during the past 5 years.The rate of extended spectrum beta-lactamase(ESBLs)in Escherichia coli was from 43.6% to 60.1%.There is no Imipenem and Meropenem resistant strain.Susceptibility test showed that Escherichia coil producing ESBLs was multiple resistant.In recent 5 years,the antimicrobial resistance of Escherichia coil has increased.Conclusion Monitoring of the antimicrobial resistance to Escherichia coil should be strengthened.The change of the antimicrobial resistance should be investigated in order to direct rational drug usage in the clinic and prevent bacterial strain of drug resistance from being transmitted.
