系统性红斑狼疮患者抗卵巢细胞抗体与月经不调的相关性研究

摘    要:目的了解抗卵巢细胞抗体与系统性红斑狼疮(SLE)患者出现月经不调的相关性。方法随机选取女性SLE患者160例,患者均未应用免疫抑制剂及大剂量激素,并排除妇科和内分泌疾病。采用酶联免疫吸附试验(ELISA)法检测血清抗卵巢细胞抗体,同时记录发病年龄、月经情况及抗核抗体(ANA)等自身抗体的测定结果。结果160例女性系统性红斑狼疮患者中,血清抗卵巢细胞抗体阳性56例,占35.0%。其中24例在未应用免疫抑制剂及激素之前即出现月经不调(占42.9%)。在104例抗卵巢细胞抗体阴性患者中,仅17例出现月经不调(占16.3%)。抗卵巢细胞抗体阳性组抗dsDNA抗体和抗核小体抗体(AnuA)的阳性率明显高于抗卵巢细胞抗体阴性组。结论SLE患者可出现疾病相关的月经不调,抗卵巢细胞抗体与月经不调的发生密切相关。

Abstract:Objective To investigate the relationship between anti-ovary antibodies and abnormal menstruation in patients suffered from SLE. Metheds 160 women with systemic lypus erythermatosus (SLE) in the department were randomly selected as the study group. Women with diseases of gynaecology and endocrine system were excluded. For each eligible patient who was not treated with steroid and immunosuppressant, the medical information was collected including age, history of menstruation, and immunological parameters such as anti-ovary antibody and ANA. Results Of the 160 women patients, the anti-ovary antibody were positive in 56. In the positive group, 24 patients developed abnormal menstruation before they were treated with regular steroid and immunosuppressant. In the negative group, only 17 patients developed abnormal menstruation. The ratio of anti-dsDNA antibody and AnuA in patients with positive anti-ovary antibody was higher than that of the patients with negative anti-ovary antibody. Conclusion Some patients with SLE develop abnormal menstruation at the beginning of disease. The ratio of abnormal menstruation in patients with positive anti-ovary antibody is higher than that of the patients lack of anti-ovary antibody.
