应用椎间盘切吸技术治疗颈椎病的临床对照研究

摘    要:目的 :探讨椎间盘切吸技术在颈椎病治疗中应用的方法和疗效。方法 :采用主要病变节段切除植骨加次要节段椎间盘切吸的方法 ,治疗多节段颈椎间盘突出症 41例 ;在单开门椎管扩大成形术或半椎板成形术的基础上 ,行后路颈椎间盘穿刺切吸 ,治疗颈椎管狭窄合并椎间盘突出症 2 2例。同传统手术治疗颈椎病的疗效进行比较。结果 :随访 3个月~ 3年 ,按 40分法评分 ,优 2 9例 ,良 2 6例 ,可 6例 ,无效 2例 ,优良率 87 3%。与传统手术组相比 ,优良率明显提高。结论 :应用椎间盘切吸技术治疗多节段颈椎间盘突出症和颈椎管狭窄合并椎间盘突出症 ,具有减压彻底、对颈椎稳定结构破坏小的特点 ,能够取得较好的治疗效果。

Abstract:Objective: To discuss the method and effectiveness of incision and aspiration in the treatment of cervical spondylosis.Methods: 41 patients were treated with anterior discectomy and fusion in the mainly pathologic segments plus cutting-aspiration in the secondary segments; 22 patientes were treated with posterior cutting-aspiration of the herniated intervertebral disc of the cervical spine after hemilaminectomy or unilateral open-door laminoplasty. The effectiveness was compared with the group treated with the traditional operations.Results: All patients were followed up with anaverage period of 27 months, the excellent and good result rate was 85.7% which was significently better than that of the traditional group.Conclusion: Application of incision and aspiration in treating complicated cervical spondylosis can decompress the cervical canal simutaneously maintain the stability, it demonstrated to be an efficacious management.
