局部麻醉在腹股沟无张力疝修补术中的应用

目的寻找更合理的腹股沟无张力疝修补术的麻醉方法。方法2003年5月至2008年5月，采用人工合成材料行腹股沟无张力疝修补手术120例，随机分两组，60例采用局部神经阻滞麻醉（局麻组），60例采用联合麻醉（对照组），分析两组的临床资料。结果局麻组手术时间平均45min。住院平均3．5d，平均花费2026元。无切口感染，无尿潴留，2例阴囊积液。联合麻醉组手术时间（加麻醉时间）平均100min。平均住院8d，平均住院费2775元。无切口感染，22例术后尿潴留，2例阴囊积液。随访12～48个月，平均36个月，两组均无复发。结论局部神经阻滞麻醉下的腹股沟无张力疝修补术是一种安全、可靠、简便、经济的方式。

Local nerve blockade anaesthesia for tension-free inguinal hernia repair

Objective To evaluate local anaesthesia for tension-free herniorrhaphy. Methods Six hundred and eighteen patients undergoing inguinal hernia repair were randomly divided into two groups received local nerve blockade anaesthesia（ 60 cases）or combined anaesthesia（ 60 cases）. Results In local anaesthesia group, the average operative time was 45 min, the average hospital stay was 3.5 days, the average cost was 2026 yuan（ RMB）, postoperative complication rate was 3.3% （2/60）, and an average 36 months follow-up found no recurrence. In combined anaesthesia group, the average operative time was 90 minutes, the average stay in hospital was 8 days, the average cost was 2775 yuan （ RMB ）, the complication rate was 40% （24 / 60 ）, and no recurrence during a follow-up of 36 months. Conclusion The tension-free groin hernia repair under local nerve blockade anaesthesia is a safe and economical method.
