高压氧对颅脑损伤患者血清MDA、SOD-1和血浆ET的影响

摘    要:目的 探讨重型闭合性颅脑损伤患者的高压氧(HBO)治疗前、后、后血清脂质过氧化物(LPO)代谢产物丙二醛(MDA)、血清超氧经物歧化酶-1(SOD)-1和血浆内皮素(ET)浓度的变化。方法 50名患者,32例为高压氧治疗组(HBO组),行HBO和其他常规治疗;18例为对照组,单纯常规治疗;30名健康志愿者为正常组。于治疗前、中、后用硫代巴比妥酸比色法检测MDA、用放射免疫法检测ET和SOD-1。结果 HBO组和对照组治疗前MDA、ET和SOD-1差异无显著性(P>0.05),但与正常组比较差异均有显著性(P<0.01);损伤两组MDA和ET与SOD-1呈负相关,MDA和ET升高和SOD-1明显降低者疗效和预后差;HBO组MDA和ET降低、SOD-1升高及总有效率均较对照组显著。结论 MDA、ET和SOD-1与损伤严重程度有关,可判断HBO的疗效和预后,并可能作为调整HBO治疗方案的依据。

Abstract:Objective The purpose of this paper was to observe the changes of malonic dialdehyde(MDA)metabolized from lipid peroxides(LPO)and superoxide dismutase(SOD 1)in serum and plasma endothelin(ET)of the patients with severe closed head injury before,during and after hyperbaric oxygenation(HBO) treatment.Methods Fifty patients were divided into two groups treated with and without HBO besides conventional treatment(HBO group and control group).There were another 30 healthy volunteers(healthy group)as normal controls.Activities of MDA were measured with spectrometry and ET and SOD 1 with radioimmunoassay.Results Before treatment there were no significant differences among the actvities of MDA,ET and SOD 1( P >0 05)in the two treatment groups,but there were significant differences where compared with the healthy group( P <0 01).There were negative correlations among MDA and ET with SOD 1 in the treatment groups.The outcome would be worse if the patients showed higher MDA and ET or lower SOD 1 after HBO treatment.There were lower MDA and ET and higher SOD 1 as well as better total treatment effect in HBO group than that in control group.Conclusions The changes of activites of MDA,ET and SOD 1 were related with the severity of brain injury and could predict the prognosis and outcome of HBO treatment of the patient and also could be an index for adjusting the scheme of HBO treatment.
