| 摘 要： | 目的 :探讨体外受精 -胚胎移植 (IVF- ET)后宫内外同时妊娠 (HP)的诊治方法以及误诊的原因。方法 :对IVF- ET后发生宫内外同时妊娠 2例进行回顾性分析。结果 :2例主要表现为确诊宫内双胎妊娠后突然出现腹痛和休克。均误诊为先兆流产 ,经剖腹探查术最终确诊为 HP,行患侧输卵管切除术 ,2例均分娩活婴。结论 :助孕技术后确诊宫内妊娠者一旦出现腹痛及腹膜刺激征或突发性休克应首先高度怀疑 HP,对 HP发病的认识不足、缺乏足够的警惕是造成延误诊断的主要原因。

| Abstract: | Objective:To investigate the diagnosis, treatment methods and misdiagnostic reasons of heterotopic pregnancy (HP) after IVF-ET.Methods:The clinical data of 2 patients with HP after IVF-ET was analysed.Results:Main clinical manifestation of two patients included sudden abdominal pain and hypovolumic shock after confirming intrauterine twin pregnancy. Two cases were misdiagnosed as threatened abortion and finally diagnosed as HP by laparotomy, and then salpingectomy was performed. Both patients delivered live infants.Conclusion:Heterotopic pregnancy must be first considered as a probable diagnosis in a patient with intrauterine pregnancy occurring abdominal pain,and peritoneal irritation sign or sudden shock after assisted reproduction technology, lacks of sufficient vigilance and pathogenetic recognition of HP are the main causes of misdiagnosis. |
