国产封堵器介入治疗房间隔缺损的临床应用及远期疗效观察

目的:总结国产封堵器经皮介入治疗房间隔缺损(ASD)的近远期疗效,评价国产封堵器治疗ASD的临床价值。方法:2002年1月-2007年12月我院应用国产封堵器介入治疗ASD患者69例,手术后分别行心电图、X线胸片和超声心动图随访。结果:68例成功,技术成功率98.6%。并发心包填塞1例,重要并发症发生率为1.4%。随访X线胸片显示肺血较术前减少,心胸比率不同程度缩小。1年后超声心动图显示右室舒张末长径由(69.3±9.7)mm变为(58.5±5.5)mm(P<0.01);右室舒张末容积由(79.1±27.3)ml变为(53.6±21.3)ml(P<0.01);肺动脉收缩压从(51.3±13.2)mmHg(1mmHg=0.133kPa)变为(29.6±9.3)mmHg(P<0.01)。全组远期随访均保持优良生存,无因ASD而再次接受治疗者。结论:国产封堵器介入治疗ASD安全可靠,创伤小、并发症少,远期疗效优良,是治疗ASD的理想方法。

Clinical Analysis on Transcatheter Closure of Atrial Septal Defect by Domestic Occluder: Intermediate and Long-term Results

Objective:To evaluate the immediate and long-term clinical effectiveness of transcatheter atrial septal defect(ASD)closure using domestic occluder.Methods: From January 2002 to December 2007,69 patients underwent transcatheter ASD closure using domestic occluder,long-term follow-up results were made with electrocardiography,chest radiography and echocardiography.Results:The technical success rate was 98.6%.Major complication was cardiac tamponade in 1 patient,and major complication rate was 1.4%.In 6~93(73.8±13.2)months follow-up,patients with enlargement of right heart were found reduced obviously(P<0.01),no significant complications occurred,none of them then underwent therapy again.Conclusion:Transcatheter closure of ASD using domestic occluder is an ideal procedure owing to its reliability and safety,with less complication and better long-term results.
