腓肠神经营养血管逆行岛状皮瓣修复足踝部软组织缺损

摘    要:
目的总结腓肠神经营养血管逆行岛状皮瓣修复足踝部软组织缺损的临床效果。方法2008年1月-2011年12月,以胭窝中点至跟腱与外踝中点的连线为轴线,外踝上5-6em为皮瓣蒂旋转点,切取范围上界不超过小腿中上1/3交界面,两侧不超过小腿侧中线,旋转点至受区创缘的最近距离为皮瓣血管蒂的长度,筋膜蒂宽至少3~4cm,设计切取腓肠神经营养血管逆行岛状皮瓣5cm×8em-13em×20em修复足踝部软组织缺损11例。结果术后3例出现皮瓣远端局部张力性水疱,皮瓣远端部分表皮坏死,经换药后愈合,余皮瓣全部成活。随访6~18月,皮瓣色泽及外观良好,负重行走正常。结论腓肠神经营养血管逆行岛状皮瓣是修复足踝部软组织缺损的理想皮瓣。

Repairing of soft tissue defects of foot and ankle with reverse island skin flap nourished by sural nerve vasa vasorum

Abstract:
Objective To summarize the clinical effects of reverse island skin flap nourished by sural nerve vasa vasorum on repairing soft tissue defects of foot and ankle. Methods From January 2008 to December 2011,11 patients with soft tissue defects in the ankle and foot were repaired by using reverse island flap nourished by sural nerve vasa vasorum. The flap area was 5 cm x 8 cm to 13 cm x 20 cm. Results After operation flaps in 3 patients had partial tension blisters and partial epidermal necrosis on distal flap, which healed through change of dressings. Others flaps survived. After 6-18 months of follow-up,skin color and appearance of the flaps were both good with normal weight-bearing ambulation. Conclusions Reverse island skin flap nourished by sural nerve vasa vasorum is an ideal one to repair soft tissue defects of foot and ankle.
