补肾益髓开窍方及VitalStim电刺激联合西药治疗脑卒中后吞咽障碍随机平行对照研究

[目的]观察补肾益髓开窍方及VitalStim电刺激联合西药治疗脑卒中后吞咽障碍疗效。[方法]使用随机平行对照方法,将60例住院患者按就诊顺序编号方法随机分为两组,对照组30例应用美国产VitalStim电刺激仪刺激,30min/次,1次/d,6次为1个疗程,共3个疗程,疗程间休息1d(VitalStim专门培训的语言治疗师操作,电流以患者耐受的强度为大小,刺激同时患者进行吞咽动作;西药常规对症治疗。治疗组30例补肾益髓开窍方(熟地15g,山药25g,山茱萸、石菖蒲各15g,远志、天麻、全蝎、僵蚕、胆南星各10g),1剂/d,水煎300mL,早晚口服;西药常规对症治疗同对照组。观测临床症状、吞咽障碍评分。6d为1疗程,连续治疗3疗程,判定疗效。随访20d(时间与例数)。[结果]治疗组痊愈12例,显效8例,有效6例,无效4例,总有效率86.67%。对照组痊愈8例,显效5例,有效6例,无效11例,总有效率63.33%。治疗组疗效优于对照组(P<0.01)。吞咽障碍评分改善治疗组优于对照组(P<0.01)。[结论]补肾益髓开窍方及VitalStim电刺激联合西药治疗脑卒中后吞咽障碍安全、有效,方便、无创、易操作,对患者的配合程度要求较低,无明显不良反应。

A Randomized,Parallel Controlled Study on BushenYisuiKaiqiao Method combined with VitalStim Electrical Stimulation for Treating Dysphagic Patients with Stroke

[Objective]To survey the effect of BushenYisuiKaiqiao method combined with VitalStim electrical stimulation for treating dysphagic patients with stroke.[Method]Sixty subacute stroke patients who were hospitalized,participated the study.All patients were divided into 2 groups:①control group(n=30)were treated with electrical stimulation with western medicine,30 min/time 1 times per day,,6 times as one therapeutic course,total 3 therapeutic course② treatment group(n=30)were treated treated with BSYSKQ recipe(consisted of 9 Chinese herbal drugs as Rehmannia glutinosa root,yamaimo,Cornus officinalis,Radix Polygalae,etc.) orally taken,2 times per day,100ml/times,6 day as one therapeutic course,total 3 therapeutic course.Before and after receiving treatment regimen,the patients were evaluated with Water-Swallow Test,Swallow Function Score and clinical curative effect assessment.[Result]All the 60 patients were involved in the analysis of results.12 cases of the treatment group were cured,8 cases have obvious effect,effective in 6 cases,4 cases are invalid,the total efficiency is 86.67%.8 cases of the control group were cured,5 cases have obvious effect,effective in 6 cases,11 cases are invalid,the total efficiency is 63.33%.The effect of the treatment group is better than in control group(P<0.01).Dysphagia score improved in treatment group than in control group(P<0.01).[Conclusion]The method of BushenYisuiKaiqiao method combined with VitalStim electrical stimulation for treating dysphagic patients is safe,effective,convenient,non-invasive,easy operation,patients with low requirements,without obvious adverse reaction
