左旋氨氯地平联合培哚普利治疗老年高血压合并左室肥厚的疗效和安全性评价

目的观察左旋氨氯地平联合培哚普利治疗老年高血压合并左室肥厚的疗效,并评价其安全性。方法 2009年12月～2010年12月,将68例老年原发性高血压合并左室肥厚患者随机分为两组,观察组34例采用左旋氨氯地平联合培哚普利治疗,对照组34例仅采用左旋氨氯地平治疗,比较观察两组的24动态血压及对舒张末期左室后壁厚度(PWT)、舒张末期室间隔厚度(IVS)、舒张末期左室腔内径(LVDd)和左心室重量指数(LVMI)的变化以及不良反应的发生情况。结果两组治疗后24h收缩压及24h舒张压水平明显下降(P﹤0.05);并且,观察组治疗后24h收缩压及24h舒张压水平明显低于同期对照组(P﹤0.05)。两组治疗后PWT、IVS、LVDd及LVMI水平明显下降(P﹤0.05);并且,观察组治疗后PWT、IVS、LVDd及LVMI水平均明显低于同期对照组(P﹤0.05)。两组不良反应发生率比较,差异无统计学意义(P﹥0.05)。结论左旋氨氯地平联合培哚普利治疗老年高血压合并左室肥厚患者进行治疗,具有协同效应,能够明显降低血压,逆转左室肥厚,不增加不良反应的发生,值得临床推广应用。

The effect of levamlodipine combined with perindopril in treatment with primary hypertension with left ventricular hypertrophy of old patients and its safety evaluation

OBJECTIVE To observe the effect of levamlodipine combined with perindopril in treatment with primary hypertension with left ventricular hypertrophy of old patients and evaluate its safety.METHODS From September 2009 to September 2010,68 cases of old primary hypertension with left ventricular hypertrophy patients were classified into two groups,which observation group of 34 cases were treated with levamlodipine combined with perindopril and control group of 34 cases were only treated with levamlodipine.The 24h blood pressure,the levels of PWT,IVS,LVDd and LVMI,and the adverse effects between the two groups were compared.RESULTS After treatment,the levels of 24h SBP and 24h DBP were significantly decreased in both groups(P﹤0.05).Moreover,the levels of 24h SBP and 24h DBP in observation group were significantly lower than those in control group(P﹤0.05).After treatment,the levels of PWT,IVS,LVDd and LVMI were significantly decreased in both groups(P﹤0.05).Moreover,the levels of the levels of PWT,IVS,LVDd and LVMI in observation group were significantly lower than those in control group(P﹤0.05).There were no significant differences in adverse reactions between two groups(P﹥0.05).CONCLUSION Levamlodipine combined with perindopril in treatment with hypertension with left ventricular hypertrophy of old patients has synergistic effects,can lower blood pressure and reverse left ventricular hypertrophy,dose not increase adverse reaction,which can be applied in clinic.
