动脉硬化闭塞性肢体坏疽截肢平面的选择

①目的探讨动脉硬化闭塞性肢体坏疽截肢手术平面的选择.②方法对42例动脉硬化闭塞症导致的下肢干性坏疽病人的截肢手术情况进行回顾性分析.根据此类疾病的临床、病理特点,经统计学处理后筛选患肢静息痛部位、坏疽部位、小腿营养性改变、动脉闭塞部位、患肢抬高及下垂试验等5项作为评价指标并制定评分标准.③结果本组病人经以上标准评分,7分者4例,6分者5例,5分者8例,4分者12例,4分以下者13例.1～4分组和5～7分组在截肢平面选择上的差异具有极显著性(x2=18.49,P＜0.01).④结论动脉硬化闭塞性肢体坏疽病人评分≥5分者宜行股部中下1/3截肢术,评分＜5分者宜行小腿中下1/3截肢术.

AMPUTATION LEVEL FOR ATHEROSCLEROTIC GANGRENE

Objective To study the amputation level in the treatment of atherosclerotic gangrene. Methods The authors reviewed 42 patients underwent amputation for dry gangrene of leg due to atherosclerosis obliterans. According to the clinical and pathological features of this illness, 5 items , i.e. region of rest pain, location of gangrene, nutritious alteration of affected legs, the part of arterial occlusion as well as the elevating and declining tests of the affected legs, were selected for evaluation and grading standards made after statistical analysis. Results According the criteria , 4 patients scored 7; 5 patients scored 6; 8 patients scored 5; 12 patients scored 4 and 3 patients scored less than 4. Conclusion The atherosclerotic gangrene patients whose scores exceed or equal to 5 points should undergo femoral amputation, for those less than 5 points , a tibien amputation be done.
