摘    要:
目的 探讨超微血管成像(SMI)技术对甲状腺结节良恶性鉴别的诊断价值.方法 对45个已进行常规超声检查的甲状腺结节术前行灰阶模式超微血管成像(mSMI)检查,观察结节在mSMI显像时结节边缘的血流显示特点,并结合病理分析mSMI在甲状腺结节良恶性判断中的运用价值.结果 mSMI对45个甲状腺结节的诊断与病理诊断的一致性较好,Kappa值为0.687,mSMI技术的诊断符合率为84.44%,高于常规超声的66.67%.结论 mSMI技术能清楚地显示甲状腺结节边缘的血流分布特点,为甲状腺结节良恶性判断提供新的依据.

Abstract:
Objective To evaluate the diagnostic value of monochrome superb microvascular imaging (SMI) for dif-ferentiating benign from malignant thyroid nodules. Method Forty-five thyroid nodules that had been examined by con-ventional ultrasound were selected and detected preoperatively using mSMI in grayscale mode. The characteristics of blood flow at the edges of nodules in mSMI image were observed, and the results and pathological data were analyzed to evaluate the diagnostic value of mSMI for the differentiation of benign from malignant thyroid nodules. Result The di-agnostic results of 45 thyroid nodules were consistent between mSMI and pathological examinations, the Kappa value was 0.687, diagnostic coincidence rate of mSMI was 84.44%, higher than the 66.67%of conventional ultrasound. Conclu-sion The characteristics of blood flow at the edges of the thyroid nodules in mSMI images are clear, and it provides a new basis for the differentiation of benign from malignant thyroid nodules.
