结直肠癌K-Ras突变的影响因素分析

目的 探讨结直肠癌K-Ras突变的影响因素.方法 以150例结直肠癌患者为研究对象,同时选取200例健康成年人为对照组.观察两组K-Ras突变率的差异,以及不同临床病理特征的结直肠癌患者K-Ras突变率,分析结直肠癌患者K-Ras突变率的影响因素.结果 结直肠癌患者的K-Ras突变率为76.67%,明显高于对照组(6.50%),差异有统计学意义(P<0.01);单因素分析显示,有淋巴结转移、组织学分型为管腺癌、TNM分期为Ⅲ+Ⅳ期、肿瘤直径≥5 cm的结直肠癌患者其K-Ras突变率较高(P<0.05);多因素Logistic回归分析显示,有淋巴结转移、组织学分型为管腺癌和TNM分期为Ⅲ+Ⅳ期是影响结直肠癌患者K-Ras突变的危险因素,其OR值分别为4.235、4.857和5.023,差异具有统计学意义(P<0.05).结论 K-Ras突变率在结直肠癌患者中较高,且与患者的组织学分型、TNM分期和有无淋巴结转移有关.

Analysis of factors influencing K-Ras mutation in colorectal cancer

Objective To explore the factors influencing K-Ras mutation in colorectal cancer. Method 150 cases of colorectal cancer patients were included in the study, at the same time, 200 healthy adults were enrolled as controls group. The K-Ras mutation rate was observed in the two groups of patients with different clinicopathological characteristics, and risk factor analysis was performed. Result The mutation rate of K-Ras in colorectal cancer patients was 76.67%, which was significantly higher than that in control group at 6.50%(P<0.01);Univariate analysis showed that, lymph node metas-tasis, histological type of ductal carcinoma, TNM stage Ⅲ+Ⅳ, tumor diameter≥5 cm were associated with higher K-Ras mutation rate (P<0.05);Multivariate Logistic regression analysis showed that, lymph node metastasis, histological type of ductal carcinoma, and TNM stage III+IV were the risk factors for K-Ras mutations, with OR values being 4.235, 4.857 and 5.023, respectively, and the differences were statistically significant (P<0.05). Conclusion K-Ras mutation rate is higher in patients with colorectal cancer, and is associated with patients'histological type, TNM stage and metastases.
