窄谱中波紫外线治疗137例219块白癜风皮损复色模式分析

摘    要:目的总结窄谱中波紫外线治疗137例219块白癜风皮损的复色模式,并分析影响复色模式的因素。方法按照常规治疗方案,使用Waldmann 801KL型紫外线皮肤治疗仪对137例219块白癜风皮损进行治疗,治疗12周后总结白癜风皮损的复色模式;采集患者基本信息及白癜风病情相关指标,与皮损复色模式进行单因素统计分析,筛选出差异有统计学意义的指标后,将上述阳性指标进行统计学建模,采用无序多分类Logistic回归分析阳性指标与复色模式的相关性。结果 137例白癜风患者219块皮损,男71例,女66例;平均年龄29岁;219块白癜风皮损治疗后,98块(44.74%)皮损出现边缘复色模式,85块(38.81%)皮损出现中央复色模式,36块(16.44%)出现混合复色模式。单因素分析结果显示,不同部位、白癜风疾病分型、白发、同形反应与复色模式相关(P<0.05);建立模型后无序多分类Logistic回归分析结果显示,白癜风疾病分型、年龄与复色模式存在相关性(P<0.05)。结论患者年龄、白癜风疾病分型是窄谱中波紫外线治疗白癜风皮损复色模式的决定因素。

Abstract:Objective To analyze repigmentation patterns in vitiliginous patches and clinicoepidemiological characteristics. Methods All patients had completed a 12-week treatment with NB-UVB, and their clinical records were reviewed. The repigmentation pattern was determined using single factor statistical analysis. Significant indicators were identified, the positive indicators were statistically modeled, and unordered multinomial logistic regression analysis was used to analyze the correlation between positive indicators and repigmentation patterns. Results A total of 219 lesions in 137 patients ( 71 females and 66 males), with the average age of 29 years old, were included in this study. Overall,98(44. 74%) lesions showed marginal repigmentation pattern, while 85 ( 38. 81 %) lesions showed perifollicular repigmentation pattern. Thirty lesions ( 16. 44%) showed mixed repigmentation pattern. Univariate analysis showed that body sites and subtypes of vitiligo, white hair, and Koebner phenomenon were associated with repigmentation patterns ( P < 0. 05 ). The results of unordered multinomial logistic regression analysis demonstrated a correlation of subtypes of vitiligo and age with repigmentation pattern ( P < 0. 05 ). Conclusion Age and the subtypes of vitiligo dominate the repigmentation pattern of vitiligo in response to the narrow-band ultraviolet radiation treatment.
