二孔法腹腔镜治疗小儿腹股沟斜疝并发症的原因分析和预防

目的:探讨腹腔镜治疗小儿腹股沟斜疝并发症的预防手段。方法:回顾分析我院2010年4月～2011年2月行腹腔镜治疗218例小儿斜疝临床资料。结果:218例小儿斜疝患者均在腹腔镜下完成疝内环口高位结扎术,每侧手术时间8～15 min,平均8 min,术后1～2 d出院。术中1例内环口与大网膜粘连严重,中转开放手术,无阴囊血肿。术中探查发现对侧隐匿性疝41例,一并行疝囊高位结扎术。术后回访6～18个月,术后无复发。结论:腹腔镜是治疗小儿腹股沟斜疝的有效手段,但需要充分的术前准备和细致的手术操作才能防止并发症的发生。

The cause analysis and prevention of complications on double-drilled technique in pediatric inguinal hernia

Objective: To explore the prevention of complications of laparoscopic technique in pediatric inguinal hernian. Methods: A retrospective analysis of clinical data of 218 cases with laparoscopic treatment for pediatric inguinal hernia in our hospital from April 2010 to February 2011.Results: 218 cases with laparoscopic treatment for pediatric inguinal hernia were done under high ligation of hernia inguinal ring,each side of the operation time was 8 ～ 15 min,the average 8 min.They were discharged from hospital after 1 ～ 2 d.Inner mouth surgery in 1 case of severe adhesion with omentum, conversion to open surgery,no scrotal hematoma.Exploratory surgery found occult contralateral hernia in 41 cases,a high ligation of hernia sac in parallel.A return visit after 6 to 18 months after surgery without recurrence.Conclusions: Laparoscopic treatment of indirect inguinal hernia is effective,but the adequate preoperative preparation and meticulous surgical operation is needed to prevent complications.
