摘    要:目的探讨AO微创内同定系统(LISS)治疗复杂性胫骨近端骨折及胫骨干多段粉碎性骨折的方法和临床效果。方法分析2005年2月至2006年6月对14例复杂性胫骨近端骨折及胫骨干多段粉碎性骨折的患者采用AO微创内固定系统治疗,术后对其伤口愈合、术前及术后X线片检查及膝踝关节功能恢复进行定期观察。结果本组14例创口全部Ⅰ期愈合,全部病例随访10~18个月,平均12个月;14例骨折完全愈合,X线片显示骨折愈合时间约为术后16~30周。无明显短缩及旋转畸形,外观无明显畸形。对照测试踝膝关节功能,按Johner-Wruhs方法评价.优10例、良2例、中2例、差0例,本组病例优良率为85.7%。结论AO微创内固定系统具有创伤小、并发症少、骨折愈合率高等优点.是治疗复杂性胫骨近端骨折及胫骨干多段粉碎性骨折的一种有效微创内固定治疗方法。

Treatment of complex fractures of tibia with Less Invasive stabilization system(LISS)

Abstract:Objective To investigate the method and clinical outcome of treating the complex comminuted fractures at proximal tibia and Shaft of tibia with AO less invasive stabilization systems (LISS). Methods The clinical data of 14 cases of complex comminuted fractures at proximal tibia and Shaft of tibia from January 2005 to june 2006 were analyzed retrospectively,All patients were treated by the internal fixation with LISS-proximal tibia. Healing of Wounds, the X-ray films before and after operations,and the recovery of ankle-knee joint function were observed regularly. Results All patients were followed up from 10 months to 18 months(average 12 months); All Fractures were eventually healed,The time of bone union was 16- 30 weeks according to the X films.No obvious deformity of crispation and revolution. According to the scoring systems of Johner-Wruhs for the function of ankle-knee joint,the results were excellent in 10 cases .good in 2 cases and fair in 2 cases,the rate of excellent and good was 85.7%. Conclusion LISS is an effective method of internal fixation for treating complex comminuted fractures at proximal tibia and Shaft of tibia.It has the advantages of less injury,few complications and a high union ratio of fractures.
