乳腺不典型导管乳头状瘤彩色多普勒超声声像图特征分析

目的 探讨乳腺不典型导管乳头状瘤二维及彩色多普勒超声声像图特征.方法 应用彩色多普勒超声诊断15例不典型导管乳头状瘤,并将30例浸润性导管癌及30例乳腺纤维瘤作为对照,将手术病理结果作为金标准,分析比较三者二维以及彩色多普勒声像图特征异同,目的在于提高彩色多普勒超声对不典型乳腺导管乳头状瘤的诊断能力.结果 本组中15例不典型乳腺导管乳头状瘤均表现为完全实性,其与导管浸润癌纵横比均接近1,二者比较差异无统计学意义(P>0.05),不典型导管乳头状瘤形态不规则检出率为53.3%,低于导管浸润癌的96.7%,二者比较差异具有统计学意义(P<0.05),而不典型导管乳头状瘤后方声衰减以及钙化均少见(检出率分别为6.7%以及20%),明显低于导管浸润癌(检出率分别70%以及80%),二者比较差异均具有统计学意义(P<0.05),而与纤维腺瘤(检出率分别为10%及30%)比较差异无统计学意义(P<0.05).不典型导管乳头状瘤以及纤维腺瘤血流分级以Alder0-Ⅰ级多见,而导管浸润癌则以AlderⅡ-Ⅲ级多见.结论 结合二维以及彩色多普勒超声声像图特征有助于提高不典型导管乳头状瘤的诊断准确性.

Analysis of Characteristics of Two-dimensional and Color Doppler Ultrasonography in the Diagnosis of Uncommon Intraductal Papilloma of the Breast

Objective To investigate the characteristics of two-dimensional and color Doppler ultrasonography in diagnosis of uncommon intraductal papilloma of the breast.Methods A retrospective analysis was performed,which included the characteristics of two-dimensional and color Doppler ultrasonography of 15 patients with uncommon intraductal papilloma of the breast,30 patients with fibro adenoma of breast and 30 patients with breast infiltrative ductal carcinoma.The results of them was verified by the operation and histopathological examinations.Results There was no statistics significance for the aspect ratio between uncommon intraductal papilloma of the breast and infiltrative ductal carcinoma of breast.The detecting rate of anomalous shape of uncommon intraductal papilloma of breast was 53.3%(8/15),lower than that of infiltrative ductal carcinoma of breast,which was 96.7%(29/30).The detecting rate of calcification and posterior acoustical shadow of solid intraductal papilloma of breast were 6.7%and 20%,lower than that of the infiltrative ductal carcinoma of breast which were 70%and 80%,but there was non-statistic significance compared with fibro adenoma of breast.The blood graduation of uncommon intraductal papilloma of breast was Alder 0-Ⅰlevels.Conclusions Two-dimensional and color Doppler ultrasonography examination can raise the diagnostic accuracy of uncommon intraductal papilloma of the breast.
