人工关节置换与加压螺钉内固定治疗高龄骨质疏松性股骨颈骨折的疗效观察

摘    要:目的对比分析人工关节置换与加压螺钉内固定两种不同的处理方法治疗高龄骨质疏松性股骨颈骨折患者的疗效,探索更为合理的老年患者骨折疏松性股骨颈骨折的治疗方法。方法回顾性分析我院2007年7月到2010年6月采用加压螺钉内固定治疗42例高龄骨质疏松性股骨颈骨折患者的治疗及预后情况,并与2010年7月到2012年7月采用人工关节置换治疗的30例高龄骨质疏松性股骨颈骨折患者的治疗及预后情况相比较。结果与加压螺钉内固定组相比,除手术时间长和出血量多外,人工关节置换组术后下地时间短,并发症较少及Harris评分得分优良率高,差异均具有统计学意义(P均<0.05)。结论人工关节置换术治疗高龄骨质疏松性股骨颈骨折患者,预后较好,并发症少,可改善患者术后生活质量。

Effect Analysis of Artificial Joint Replacement in the Treatment of Elderly Patients with Osteoporotic Femoral Neck Fracture

Abstract:Objective To analyze the effect of artificial joint replacement in the treatment of elderly patients with osteoporotic femoral neck fracture. Methods The clinical data of 42 elderly patients with osteoporotic femoral neck fracture in our hospital from July 2007 to June 2010 treated with compression screw internal fixation and 30 elderly patients with osteoporotic femoral neck fracture from July 2010 to July 2012 treated with artificial joint replacement were analyzed retrospectively. The surgical situation and prognosis were compared between two methods. Results Compared with compression screw internal fixation group, the artificial joint replacement group had shorter operating time, less blood loss, shorter postoperative ambulation time, fewer complications and higher Harris score; the difference between the two groups was statistically significant (all P 〈0.05). Conclusions Artificial joint replacement has good prognosis and fewer complications in the treatment of elderly patients with osteoporotic femoral neck fracture, which can improve patients' quality of life.
