腹腔镜结肠癌根治术临床疗效评估

目的]探讨张力蛋白同源（PTEN）、环氧化酶-2（ COX-2）及基质金属蛋白酶9（MMP-9）在骨肉瘤中的表达及其意义.方法]选取骨肉瘤石蜡包埋标本28例,以25例骨软骨瘤石蜡包埋标本作为对照.采用SP法对以上病理标本进行免疫组化染色.检测PTEN、COX-2及MMP-9的表达情况.结果]PTEN在骨肉瘤组织中的表达率为57.1%（16/28）,显著低于其在骨软骨瘤组织中的表达率为88.0%（22/25）,两者的差异有显著性（P〈0.05）;COX-2和MMP-9在骨肉瘤中的表达率78.6%（22/28）及82.1%（23/28）,显著高于在骨软骨瘤中的表达率分别为36.0%（9/25）和28.0%（7/25）,差异有显著性（P〈0.05）.PTEN在不同分期以及不同病理亚型骨肉瘤组织中的阳性表达差异无显著性（P〉0.05）.COX-2及MMP-9表达与病理亚型无明显相关,而与临床分期关系密切.PTEN与COX-2和MMP-9的表达具有负相关性;COX-2与MMP-9的表达具有正相关性.结论]PTEN、COX-2及MMP-9与骨肉瘤的发生、发展有相关性,联合检测对于骨肉瘤的诊断和预后判断具有一定的意义.

Evaluation of the Clinical Effect of Radical Resection of Colon Cancer by Laparoscopy

Objective] To evaluate the clinical effect of radical resection of colon cancer by laparoscopy. Methods] Sixty cases of colon cancer were divided into laparoscopy group（n= 28） undergoing laparoscopic radical resection and laparotomy group（n= 32） undergoing open radical resection. The state during operation was compared between two groups. Results]All 28 cases in laparoscopy group achieved successfully. There was no significant difference in the operation time, mean number of excised lymph node, negative incisal edge and relapse rate between two groups（ P 〉0. 05）. The intraoperative blood loss and the length of stay in laparoscopy group were less than those in laparotomy group, and there was significant difference between two groups （ P〈0.01）. After one-year postoperative follow-up, the quality of life in laparoscopy group was better than that in laparotomy group. Conclusion]Laparoscopie radical resection of colon cancer is safe, feasible, effective and minimally invasive.
