2型糖尿病患者血清脂联素、超敏C-反应蛋白与胰岛素敏感性相关性分析

摘    要:
[目的]探讨2型糖尿病(T2DM)患者血清脂联素、超敏C-反应蛋白(hs-CRP)与胰岛素敏感性相关性。[方法]测定2型糖尿病组、T2DM合并肥胖组、单纯性肥胖组及正常对照组空腹血糖(FPG)、空腹胰岛素(FINS)、脂联素及超敏C反应蛋白(hs-CRP),计算胰岛素抵抗指数(HOMA-IR)及胰岛素敏感指数(ISI);前3组给予罗格列酮8周治疗后,重复测定,并对前后数据进行统计学比较。[结果]2型糖尿病及肥胖较正常对照组FINS、hs-CRP、HOMA-IR升高,脂联素、ISI降低;给予罗格列酮8周后,FINS、hs-CRP、HO-MA-IR明显下降,脂联素、ISI明显升高,差异有统计学意义;hs-CRP与FPG、FINS、HOMA-IR呈正相关,与脂联素、ISI呈负相关。[结论]血清标志物hs-CRP、脂联素的检测,对2型糖尿病早期诊断、治疗、预后、胰岛素抵抗及生理机制的研究具有重要意义。

Abstract:
[Objective] To investigate the correlation between adiponeetin, supersensitive C reaction protein (hs-CRP) and insulin sensitivity in type 2 diabetic patients. [Methods] FPG, FINS, adiponectin, hs-CRP were measured in the type 2 DM, type 2 DM with obesity, simple obesity and normal control group. Insulin resistant index (HOMA-IR) and insulin sensitable index (ISI) were calculated. These figures were measured again in the experiment group after treated with rosiglitazone (RSG) for 8 weeks. All the data were evaluated statistically. [Results] The FINS, hs-CRP, HO- MA-IR increased while the adiponectin, ISI decreased in the DM and obesity as compared with the control. The FINS, hs-CRP, HOMA-IR decreased obviously after treated with RSG 8weeks. Meanwhile, adiponectin, ISI increased significantly. The difference was statistically significant. The hs-CRP had correlation to FPG, FINS, HOMA-IR and with reverse correlation to adiponectin and ISI. [Conclusions] Detection of hs-CRP, adiponectin regarded as the marker of type2 DM, has significant value for the diagnosis, treatment, prognosis and the study of the mechanism of insulin resistance.
