某肿瘤专科医院2009-2011年麻醉药品应用分析

目的:评价某肿瘤专科医院麻醉药品的应用现状及趋势。方法:收集该院2009-2011年麻醉药品的消耗数据,对其品种、用量、用药频度(DDDs)、药物利用指数(DUI)、销售金额及比例等进行统计、分析。结果:该院麻醉药品销售金额及用量均呈逐年增加趋势。其中,吗啡制剂用量始终居首位,羟考酮缓释片用量增长迅速;芬太尼贴剂的DDDs列首位,年均2万人次,芬太尼注射液及其衍生物销售金额居首位;哌替啶的用量逐年下降,每年减少约30%,已极少用于治疗癌症疼痛;口服制剂用量和金额均大大高于注射剂型;吗啡口服缓释制剂DUI均>2,显著高于其他制剂;芬太尼贴剂的DUI也超过了1.5。结论:该院麻醉药品注射剂型用法用量合理,但吗啡缓释制剂的用量较大且DUI偏高,可能存在过量应用情况;芬太尼贴剂用量也较大,应进一步加强对其合理使用监管。

Analysis of Narcotic Drugs Use in Our Hospital during 2009--2011

OBJECTIVE： To evaluate the current situation and trend of narcotic drug use in our hospital. METHODS： The con- sumption data of narcotic drug in our hospital during 2009- 2011 were collected and analyzed statistically in terms of types, amount, DDDs, DUI, consumption sum and proportion. RESULTS ： The consumption sum and amount of narcotic drugs increased year by year. The amount of morphine took up the first place, but Oxycodon sustained-release tablet increased rapidly; Fentanyl patches ranked the first in the list of DDDs with twenty thousand persons per time annually. Fentanyl injection and its dearivant ranked the first in the list of consumption sum; the amount of pethidine decreased year after year, decreasing by about 30% in each year, and it was seldom or never used for cancer painp; the amount and consumption sum of oral preparations were much higher than the injection; DUI of morphine oral sustained-release preparations were greater than 2, which was significantly higher than the other preparations; DUI of Fentanyl patch were more than 1.5. CONCLUSION： The usage and dosage of narcotic drugs in- jection are reasonable, but a large amount of Morphine sustained-release preparations are used and the DUI is large, there may be excessive use of drugs; the amount of Fentanyl patches is also large, we should further strengthen supervision of rational use of it.
