中药浴配合推拿治疗痉挛型脑瘫患儿踝关节活动障碍的临床观察

目的：观察中药浴配合推拿治疗痉挛型脑瘫患儿踝关节活动障碍的短期疗效。方法：治疗前测定患儿足背屈角，并进行综合痉挛量表（CSS）评分。采用药浴加推拿方法，作用于小腿三头肌及踝关节，结束后测定患儿足背屈角及CSS评分，并评定其短期疗效。结果：治疗前后患儿踝关节活动度有明显改善（P〈0．05），有效率为85．5％，CSS评分较治疗前明显降低（P〈0．05），有效率为70．0％。结论：采用中药浴配合推拿治疗痉挛型脑瘫患儿踝关节活动障碍，短期内可明显改善踝关节活动度及小腿三头肌痉挛，可作为痉挛型脑瘫临床辅助治疗及家庭康复治疗的有效手段之一。

Clinical observation of ankle dyscinesia of cerebral palsy treated by Chinese herb bath combined with massage

Objective:The ankle dyscinesia of cerebral palsy interfered the rehabilitation of stand and walk of the children with cerebral palsy. The study was designed to observe the short-term curative effects of the Chinese herb bath combined with Chinese traditional massage. Methods:40 children with cerebral palsy of spastic type,altogether 76 ankles. The musculus triceps surae and ankle were exerted by herb bath and massge. The angle of dorsiflexion of foot were measured and CSS were evaluated before and after the treatment. Results: There were obvious improvement of the ankle of dorsiflexion of foot (P < 0.05) after the treatment. The effective rate was 85.5 %. And the score of CSS decreased obviously after the treatment. The effective rate was 70 %. Conclusion:The Chinese herb bath combined with massage can obviously improve the ankle range-of-motion and relieve the spasticity of the musculus triceps surae in short-term. It can applied to the clinical auxiliary treatment and home rehabilitation of cerebral palsy of spastic type as one of the effective methods.
