紫龙金对鼻咽癌放射增敏作用研究及机制探讨

摘    要:目的:紫龙金对鼻咽癌放射增敏作用实验及临床研究,以及增敏机制的探讨。方法:离体研究采用集落形成分析法研究中药紫龙金对鼻咽癌细胞系CNE-I放射增敏作用。采用多靶单击模型拟合细胞存活曲线,并计算出D0、Dq值、常规照射2戈瑞(Gray,Gy)时细胞存活分数(survival fraction,SF2)及放射增敏比(sensitization enhancement ratio,SERD0、SERDq、SERSF2)。临床研究随机将鼻咽癌病例分为两组,放射增敏组在放疗的同期给予口服紫龙金。治疗前及放疗20GY/10f时分别予鼻咽部病理活检,行P16和cyclinD1免疫组化。结果:离体研究单纯照射组D0、Dq、SF2分别为1.44Gy、1.7Gy、0.7。照射加紫龙金组D0、Dq、SF2分别为1.03Gy、1.36Gy、0.58。紫龙金对CNE-I有较强的放射增敏作用,其SERD0、SERDq、SERSF2分别为1.40、1.25、1.20。临床研究表明紫龙金在鼻咽癌放疗增敏效果显著,增敏组放疗结束时CR率远远高于对照组。与对照组相比,增敏组P16有上调趋势,但无统计学差异;cyclin D1有下调趋势,具有统计学差异。结论:紫龙金对鼻咽癌有放射增敏作用,增敏作用机制可能是通过P16上调和cyclin D1下调。

Abstract:Objective:To investigate radiosensitization and mechanism of Chinese medicine zilongjin on human nasopharyngeal carcinoma. Methods:Cologenic assay was used to evaluate the radiosensitizing effect of zilongjin on nasopharyngeal cancer cell line CNE-I. The growth inhibition of zilongjin on CNE-I was investigated by cell account.Colony forming assay was used for survival fraction analysis. Single hit multi target model was used to plot survival curve. Clinical study was randomly divided into two groups,patients in Radiosensitizing group were given oral Zilongjin when radiotherapy.Nasopharyngeal biopsy with P16 and cyclinD1 immunohistochemistry was performed before treatment and When the total dose of radiotherapy was 20GY/10f. Results:The zilongjin has significant growth delay,the growth rate is 63%of control group on third day,the inhibition rate is 63% on sixth day. The value of D0、Dq、SF2 of radiation treatment alone was 1.44 Gy、1.7 Gy and 0.7 respectively. That of radiation treatment plus zilongjin was 1.03 Gy 、1.36 Gy and 0.58 respectively. The radiosensitivity of zilongjin on CNE-I was significant,the SERD0、SERDq and SERSF2 wete 1.40、1.25 and 1.20 respectively. Clinical study showed zilongjin is a potential sensitizer,the CR rate of sensitizing group is far higher than that of control broup at the end of radiotherapy. Compared with control group,sensitizing group has a up P16,(no significant difference)and down cyclinD1 trend ( significant difference).Conclusion:The zilongjin is a radiosensitizer for nasopharyngeal cancer. The mechanism of radiosensitization may be through upregulating P16 and downregulating cyclinD1.
