2型糖尿病患者血糖状态与颈动脉病变的关系研究

目的:研究2型糖尿病患者其血糖状态与颈动脉病变的关系。方法:选择病程为5～10年的2型糖尿病患者298例,根据空腹血糖(FPG)、餐后2 h血糖(PPG)、糖化血红蛋白(HbA1c)水平分为两组,一组为血糖控制良好组(144人),另一组为血糖控制不良组(154人),所有病例均接受颈动脉彩超常规检查,血清生化检查(肝功、肾功、血脂分析、C反应蛋白、血清脂蛋白),尿微量白蛋白,并进行体重指数(BMI)、血压(BP)的检查,比较各组间颈动脉病变情况及实验室检查结果。结果:颈动脉内膜中层厚度(IMT)随着血糖水平的升高而增加,且差异有统计学意义(P<0.05)。结论:2型糖尿病患者血糖控制差者存在更多高血压、血脂代谢紊乱等心、脑血管危险因素,血糖水平与颈动脉粥样硬化的发生、发展密切相关,持续而有效的血糖控制为2型糖尿病患者避免动脉粥样硬化发生、延缓动脉粥样硬化进展有重要意义。
